WHAT IS A RESTRICTED CERTIFICATE OF IDENTITY ?

A restricted certificate of identity is a travel document issued to permanent resident of Malaysia who are stateless

This document is for the purpose of traveling between West Malaysia and Singapore only.

The validity of the document is 5 years from the date of issue and can be renewed for another 5 years.

HOW DO I APPLY FOR A RESTRICTED CERTIFICATE OF IDENTITY ?

Application for restricted certificate of identity can be made at the Immigration Department of Malaysia by submitting the following documents :

  • Form I - IM. 45
  • 2 photographs of applicant measuring 3.5cm x 5.0cm
  • Payment of RM$300.00 for 5 years
  • Payment for renewal RM$60.00 per year

and

  • Identity card and entry permit (for first application) for applicant 18 years and above
  • Identity card, marriage certificate or divorce certificate and entry permit for applicant below 18 years and married or divorced
  • Identity card, birth certificate, identity card of father, entry permit for applicant 12 years and above and below 18 years, where father still alive
  • Identity card, birth certificate, death certificate of father, identity card of mother and entry permit for applicant 12 years and above and below 18 years, where father deceased
  • 2 photographs of applicant, identity card, birth certificate, death certificate of parents, statutory declaration of guardianship and entry permit for applicant 12 years and above and below 18 years, where parents deceased
  • 2 photographs of applicant, identity card, birth certificate, identity card of mother, entry permit, court's order on custody of child for applicant 12 years and above and below 18 years, where parents separated and father cannot be contacted.

Application for renewal

  • Certificate of identity and identity card for applicant 12 years and above or
  • Certificate of identity, birth certificate or adoption certificate for applicant below 12 years

Where you wish to apply for change of personal particular on the certificate, the following conditions apply :

  • Letter of application
  • Supporting letter from relevant agency
  • Change of information form
  • Certificate of identity
  • Identity card
  • Birth certificate
  • Marriage certificate
