HOW DO I APPLY FOR A MALAYSIA RESTRICTED PASSPORT ?

Application can be made at your nearest Immigration Department by submitting the following documents :

  • Form D - IM. 50
  • Form A1/B1 (father's permission if relevant)
  • 2 photographs of applicant measuring 3.5cm x 5.0cm
  • Old passport (if any)
  • Payment of RM$150.00 for 5 years

Malaysia restricted passport is only valid for 5 years from the date of issue and can be renewed for another 5 years.

HOW DO I RENEW MY MALAYSIA RESTRICTED PASSPORT ?

Application for renewal of your restricted passport can be made at the Immigration Department of Malaysia.

The Malaysia restricted passport can be renewed for another 5 years.

You have to submit the following documents to the Immigration Department for processing :

  • Form D - IM. 50B
  • Malaysia restricted passport
  • Identity card (if any)
  • Birth certificate (if below 12 years)
  • Adoption certificate (if relevant)
  • Photocopies of identity card, birth certificate, adoption certificate
  • Payment of RM$30 per year
