HOW DO I APPLY FOR A PASSPORT ?

Application for a passport can be made at your nearest Immigration Department by submitting the following documents :

  • Form A - IM. 42 - Pin 1/1997
  • Form A1/B1 (father's permission if relevant)
  • 2 photographs of applicant measuring 3.5cm x 5.0 cm
  • Old passport (if any)
  • Payment of
    • RM$300.00 for 32 pages or
    • RM$600.00 for 64 pages

Your passport is valid for 5 years from the date of issue and cannot be renewed. You have have to apply for a new passport thereafter.

WHAT SHOULD I DO IF I LOST MY PASSPORT ?

Where you lost your passport, you should first make a police report at the nearest police station and then report to the Immigration Department of Malaysia or Embassy of Malaysia.

Thereafter, you can apply for a replacement by submitting your application at the nearest Immigration Department together with the supporting documents, photostat copy of the police report and any other relevant document evidence.

Approval for the application of a replacement will depend on the decision of the panel.

An emergency certificate can be issued to citizen of Malaysia if you lost your passport abroad, repatriate to Malaysia on government expenses or in cases passport cannot be issued within the stipulated period.

The emergency certificate is only valid for a single journey to the country mentioned in it.

To apply for emergency certificate, you have to submitted the following documents :

  • Form F - IM. 41
  • Documentary evidence of Malaysian status
  • 2 photographs of applicant measuring 3.5cm x 5.0cm
  • Payment of RM$50.00

and

  • Identity card or police or army identity card for applicant 18 years and above
  • Identity card and marriage certificate for applicant below 18 years and married
  • Identity card, if any, birth certificate and identity card of father for applicant below 18 years, where father is still alive
  • Identity card, if any, birth certificate, death certificate of father and identity card of mother, where father deceased
  • Identity card, if any, birth certificate, death certificate of parents and statutory declaration of guardianship, where parent deceased
  • Identity card, if any, birth certificate, identity card of mother and court's order, where parents separated and father cannot be contacted
  • Identity card, if any, identity card of legal guardian and certificate of adoption certifying child's citizenship status
