Lost or Stolen Credit Card Notice

Tips

Have you ever called your credit card company to report your lost card and no action was taken thereafter? You called again and nobody admitted receiving your call?

When you lost a credit card, the most important thing you should do is to contact the credit card company immediately to stop the card from any illegal usage. Always write down the name of the person to whom you made the report or spoken to in the credit card department including the date and time. Send this follow up letter to the credit card company.

Enter the Full Name of the person you've spoken to from the Credit Card Company and the date and time (For Example : Mr. Suhaimi Ahmad and 1st June 2000 at 12.00pm) Name Date and Time
Enter the Full Address of the Credit Card Company (For Example : No. 1, Jalan P.J., 53000, Petaling Jaya, Malaysia) Company Name Street City and State Zipcode and Country
Enter your Credit Card Type and Number (For Example : 5400-0000-0000-0000 and American Express, MasterCard or Visa) Credit Card Number Card Type
Enter the full date this document was created (For Example : 2nd June, 2000)
Enter the full date and time the Credit Card was lost (For Example : 1st June, 2000 at 11.00 am)
Enter the circumstances you lost your Credit Card [Describe situation] (For Example : I was shopping in the ABC complex with a friend. Someone snatched away my handbag from behind. A police report has been made and I am enclosing herewith a copy of the said report for your reference)
Enter the information on how you would like to collect your replacement card (For Example : send the new card to my above address or hold the card for collection at your Card Center in Petaling Jaya)
